We offer community membership pricing to qualifying low-income individuals.
If you are experiencing financial hardship and are unable to afford membership, please contact our office directly to discuss options.
If you decide to cancel, please give us 30 day's notice in writing. If you decide to re-join, you will be be charged the enrollment fee again.
What does membership cover?
Convenient access to your personal doctor, including extended office visits, e-mail, secure messaging, phone, and video chat.
Annual physicals and well person exams
Sick and urgent visits
Management of chronic diseases such as diabetes, high cholesterol, and hypertension
Preventive care including cancer screening
Depression and anxiety management
Gynecological care
In-office testing such as EKGs, pulse oximetry, urinalysis, rapid strep tests, pregnancy tests, wet mount, and KOH prep
In-office procedures including suture removal, IUD placement and removal, ear wax removal, abscess drainage, skin biopsies, trigger point injections, and cryotherapy
Weight management
Health education and coaching
For the convenience of our established patients, we offer in-clinic dispensing of commonly used medications at deeply discounted rates.

What is not part of membership?
It's important to understand that a direct primary care membership is not the same as having health insurance, and it does not take the place of health insurance.
Membership covers most primary care services that we are able to offer in the office. It does not cover hospital or ER visits, specialist care, surgery, or radiology services.
While we understand that health insurance is not affordable for everyone and we will work with you to try to find affordable options for care, we recommend that you carry insurance to cover large unexpected expenses.
